Supporting people with disabilities as they navigate complex housing systems, prepare for successful tenancy, and maintain stable housing over time.

Launched at Mainstay in 2026, this service is formally known as Housing Transition and Tenancy Sustaining Services (HTTSS).

Housing Transition and Tenancy Sustaining Services (HTTSS)

HTTSS supports people with disabilities to find and maintain housing in a private home they own, rent, or lease, separate from their service provider. This person centered service is designed to support successful housing transitions and long term housing stability.

HTTSS begins with an assessment of a person’s housing needs, preferences, and goals. From there, a housing plan is developed in collaboration with the person and their chosen team. Services are coordinated with the Individual Support Plan (ISP) and other supports to promote stability, independence, and choice.

HTTSS is designed to complement natural supports and other waiver services, and to evolve as an person’s housing needs change over time.

Mainstay assists people in finding, securing, and moving into permanent, independent housing.

Housing Transition Services may include support with:

  • Exploring housing options and completing housing applications
  • Identifying and addressing barriers to securing housing
  • Developing a housing plan based on individual needs and preferences
  • Coordinating with Supports Coordinators, landlords, and other providers
  • Preparing for move in, including tenant education and understanding lease requirements
  • Identifying community resources related to furnishings, utilities, and financial supports

Tenancy Sustaining Services focus on:

  • Supporting people to understand and comply with lease terms
  • Developing and updating housing support and crisis plans
  • Building skills related to being a successful tenant
  • Supporting communication with landlords or property managers, as needed
  • Identifying and addressing issues that could jeopardize housing stability
  • Supporting ongoing housing recertification processes, when applicable
